Event Registration

I, on behalf of myself, my child(ren), and our respective heirs and assigns, authorize Word of Faith Family Worship Cathedral (WOF) to provide transportation services for myself or my minor child(ren) in connection with Ignite Youth Events. I voluntarily release, waive, discharge, and hold harmless WOF from all claims, actions, or losses for bodily injury, property damage, wrongful death, loss of services, or otherwise, which may arise as a result of any negligent act or omission in the provision of transportation services.

I certify that I have informed WOF of any allergies that I or my minor child(ren) may have, including but not limited to allergies to gluten, nuts, latex, dairy, etc. I understand that while WOF will make every effort to provide food alternatives, snacks or meals may be withheld if suitable accommodations cannot be guaranteed to prevent allergic reactions.

I expressly agree and promise to accept and assume all risks associated with participation in Ignite Youth Ministry activities on behalf of myself or my minor child(ren).
